After installation - toolbar is missing

Hello,

I did my custom install step by step this guide: http://edndoc.esri.com/arcobjects/9....1a3824474a.htm

Problem is - when I open ArcGIS I can't see my toolbar in the toolbar menu. Only way how to solve it is: Customize->Add from File->Open "program.tlb"... after that toolbar is loaded in the toolbar menu. Funny thing is - when I open program.tlb then uninstall the program (the toolbar disappears) and after the second installation it is present in the toolbar menu :)

So what did I wrong? I need to have my toolbar in toolbar menu immediately after the first installation.
